How it works

Scan Form

The user scans the form, which is then automatically identified out of Polyform’s partner forms. The service also provides a directory of available forms.

Auto/Manual Fill

The form is automatically filled with the user’s profile information. Data is pulled from multiple profiles, including family members. The form is signed electronically.

Send, Print, Share

The final form is presented to the user and is ready to be shared, printed, etc. The partner form may be pre-configured to be sent to a specific email address, sent to a local printer, to a specific branch, etc.
